01. Scope of this policy
This policy covers information collected through the Apex website, contact forms, consultation requests, scheduling, coordinator communications, and related business operations. When information you submit is protected health information, the Apex HIPAA Notice of Privacy Practices also applies.
02. Information Apex collects
Apex may collect:
- Contact details such as your name, email address, phone number, and city or state.
- Consultation and intake details such as goals, symptoms, medication history, uploads, and other information you choose to provide.
- Operational data such as device, browser, IP address, approximate geolocation, pages visited, and website performance data.
- Payment or billing details when a transaction is processed through an approved vendor.
03. How Apex uses information
Apex uses information to operate the site, respond to inquiries, schedule consultations, provide care coordination, support telehealth services, process payments, improve the site, comply with law, and protect the security and integrity of the practice.
- To communicate with you about consultations, scheduling, service availability, or follow-up.
- To verify launch-state eligibility and local service-area coverage.
- To maintain records, troubleshoot issues, and prevent fraud, misuse, or unauthorized access.
04. Cookies, analytics, and tracking
Apex may use cookies or similar technologies for functionality, security, analytics, and service improvement. Apex does not use consultation or intake content for ad targeting, and protected health information should never be sent to advertising pixels or similar marketing trackers.
05. When Apex shares information
Apex shares information only when needed for business or care operations, to comply with law, or with your direction or authorization.
- With service providers who support hosting, scheduling, communications, payments, analytics, labs, pharmacies, or secure recordkeeping.
- With clinicians, coordinators, or business associates who need the information to support care or operations.
- When required by law, legal process, public health obligations, or to protect rights, safety, and security.
06. Retention and security
Apex uses administrative, technical, and physical safeguards designed to protect information against loss, misuse, and unauthorized access. No system can promise perfect security, but Apex limits access to people and vendors with a legitimate need to know and retains information only for as long as reasonably necessary for care, operations, compliance, and dispute resolution.
07. Your choices
Depending on the context, you may request access to information you submitted, ask for a correction, opt out of non-essential communications, or ask Apex to stop using a particular communication channel where practical.
To make a request, contact hello@apexhealth.center. Do not send sensitive clinical details over unsecured email unless Apex specifically instructs you to do so.
08. Children and updates
Apex services are intended for adults and are not directed to children under 18. Apex may update this policy from time to time. The current version will remain posted on this page with its effective date.
