01. Permitted uses
Apex uses protected health information (PHI) to provide treatment, arrange payment for services, and run the operations required to deliver care. Examples include clinical review, prescribing decisions, lab coordination, and coordinator outreach on your behalf.
02. Disclosures
PHI may be disclosed to business associates bound by HIPAA-aligned agreements, including secure communications, pharmacy partners, lab providers, and payment processors. Apex does not sell PHI and does not use it for marketing without explicit authorization.
03. Your rights
You have the right to request access to your PHI, ask for corrections, request restrictions, receive an accounting of disclosures, and file a complaint with Apex or with the U.S. Department of Health and Human Services Office for Civil Rights.
04. Reaching Apex
Contact your coordinator or submit a secure intake message to exercise any of the rights above. Do not send detailed PHI over unsecured email.
