01. Our uses and disclosures for treatment, payment, and operations
Apex may use and disclose protected health information to provide treatment, obtain or support payment, and run health care operations. Examples include clinical review, care-team communication, telehealth documentation, prescription fulfillment, lab coordination, quality review, secure storage, and billing support.
02. Other ways Apex may use or disclose information
Apex may also disclose information when permitted or required by law, including for public health activities, health oversight, law enforcement requests that satisfy HIPAA, abuse or neglect reporting, judicial or administrative process, workers' compensation, specialized government functions, or to avert a serious threat to health or safety.
Apex may share protected health information with business associates that support the practice, such as secure technology vendors, pharmacies, laboratories, payment vendors, and recordkeeping partners, provided they are required to protect the information.
03. Uses that generally require your written authorization
Apex will obtain your written authorization for uses or disclosures that require it under HIPAA, including most uses of psychotherapy notes, most marketing uses that require authorization, and the sale of protected health information. If you authorize a use or disclosure, you may revoke that authorization in writing, subject to actions already taken in reliance on it.
04. Your rights regarding your protected health information
You have rights under HIPAA, subject to certain limits and procedures.
- Request inspection or a copy of your protected health information.
- Ask Apex to amend information you believe is incomplete or incorrect.
- Request confidential communications, such as asking Apex to contact you in a different way or at a different location.
- Ask for restrictions on certain uses or disclosures, understanding Apex is not always required to agree.
- Receive an accounting of certain disclosures.
- Receive a paper copy of this notice upon request.
05. Apex duties
Apex is required by law to maintain the privacy of protected health information, provide you with this notice, follow the terms of the notice currently in effect, and notify you following a breach of unsecured protected health information when required by law.
06. Complaints and contact
If you believe your privacy rights have been violated, you may file a complaint with Apex or with the U.S. Department of Health and Human Services Office for Civil Rights. Apex will not retaliate against you for filing a complaint.

07. Changes to this notice
Apex may change this notice and make the revised notice effective for information already held and information received in the future. The current version will remain posted on this page.
